FYLDE PRIVATE MEDICAL IS COMMITTED TO PROTECTING AND RESPECTING YOUR PRIVACY
• Patient List Requirement
Our services are only available to those on our Private Patient List. A valid subscription is required to book any appointment with us.
• Patient List Fee
Is subject to a recurring monthly or annual fee. The applicable fee will be displayed clearly at the point of sign-up.
• Free Trial
We may offer a free trial period for new members. The length and terms of any free trial will be displayed at the point of sign-up. At the end of any free trial, unless you cancel your subscription we will automatically continue and the monthly fee will apply.
• Appointment Charges
Our Private Patient List fee covers access to make bookings with us but does not cover the cost of appointments. Appointments are charged separately at the rates in force at the time of booking.
• Cancellation
Subscriptions are provided on a rolling monthly or annual basis and can be cancelled at any time. If you cancel, your private patient list subscription (and access to make bookings) will end at the conclusion of your current billing period. Any booked appointments remain chargeable at the applicable rates.
• Refunds
Our Private Patient List fee is non-refundable, except where required by law. This does not affect your statutory rights.
• Individual Use
Our Private Patient List fee is provided on an individual basis and may not be transferred or shared. The only exceptions are our legacy plans — including ESSENTIAL, PREMIUM, PLATINUM, Couple, and Family Memberships — which will continue in line with their original terms. These plans are no longer available for new members as of 30th September 2025.
• Changes to our Private Patient List
We may update our Private Patient List fee, terms, or services from time to time. If we do so, we will provide reasonable notice before any change takes effect.
We are committed to ensuring that your privacy is protected. We strive to ensure your data is kept safe and that we comply with the legal obligations of the Data Protection Act 2018 (the ‘2018 Act’) and the EU General Data Protection Regulation (‘GDPR’). We will comply with the General Data Protection Regulation ((EU) 2016/679) (“GDPR”) unless and until the GDPR is no longer directly applicable in the UK, together with any national implementing laws, regulations and secondary legislation as amended or updated from time to time in the UK, and any successor legislation to the GDPR and the DPA (together “Data Protection Legislation”). This Privacy Policy sets out how we collect personal information from you and how the personal information you provide will be processed by us. What information do we hold and how will we use it?
Information you give: You may give us information about you by completing inquiry forms on the website. The information you give us may include your name, email address, address/location, and phone number. We will ask you to confirm that you give us permission to contact you.
We will retain this information while we are corresponding with you or providing services to you or to a Patient you represent. We will retain this information for as long as you or the Patient you represent is registered with us.
You may also give us your personal medical information that is stored securely in your patients notes on our medical operating system.
Fylde Private Medical will use your personal information for the following purposes only:
• to identify you when you contact us;
• to help identify services and/or products which you could have from us or selected partners;
• to help administer, and contact you about improved administration of any services and products we have provided, provide now or in the future;
• to carry out marketing analysis and customer profiling, to conduct research, including creating statistical information;
• to help to prevent and detect fraud or loss;
• to contact you in any way (including mail, email, telephone, or text) about products and services offered by us and/or selected partners unless you have previously asked us not to do so;
• to keep you up to date with our Membership Plans and Private Patient List fee, under which we will give you information and details of discounts and offers negotiated on behalf of our patients.
We may allow other people and organisations to use information we hold about you if we have been legitimately asked to provide information for legal or regulatory purposes or as part of legal proceedings or prospective legal proceedings. We will, however, always use every reasonable effort to ensure sufficient protections are in place to safeguard your personal information.
We may monitor and record communications with you (including phone conversations and emails) for quality assurance and compliance reasons.
Where you give us information on behalf of someone else, you confirm that you have provided them with the information set out in these provisions and that they have not objected to such use of their personal information. Where you give us sensitive data about yourself or another person (such as health details) you agree (and confirm that the other person has agreed) to our processing such information in the manner set out in these provisions.
We use Stripe to process private patient list subscriptions, memberships and appointment payments securely. When you enter your payment details, these are transmitted directly to Stripe. Stripe collects and processes card details on our behalf in order to manage payments and recurring subscriptions. We do not have access to or store your full payment card details.
Cookies
The Website uses cookies to distinguish you from other users of the website. Cookies are useful because they allow a website to recognise a user’s device. You can choose to accept or decline cookies. We don’t hold any information from these.
Where we are processing personal data we have obtained via the website on the basis of having obtained consent from you, you have the right to withdraw your consent to the processing of your personal data at any time. Access to patient medical records is strictly monitored and secured. Our Medical system, Meddbase (MMS), is NHS Data Security and Protection Toolkit (DSP Toolkit) compliant, ISO27001 certified and follows the strict information handling requirements of these standards.
The practice may share information with other professionals involved in your medical care, for example to enable a referral to a specialist, with your consent.
We take appropriate technical and organisational measures to protect your personal information. Payment details are processed securely by our provider, Stripe. Stripe is PCI-DSS compliant and card details are encrypted and tokenised. We do not have access to, or store, your full payment card details.
If any of the information you provide to us via the website changes, please let us know as soon as possible so that we can make the necessary changes to the information we hold for you on our database. i. General exclusions - Our services do not provide for:
· The costs associated with blood tests and diagnostic services
· The costs associated with the purchase of any medicines prescribed
· The costs incurred by consultations/treatments with any specialists referred to
· The costs associated with certain medical procedures (see price list for associated discounts)
· The cost of any vaccinations
· The cost of medical consumables
· The cost of missed appointments
· The cost of minor surgical procedures
· Consultations for any non-members
2. Additional Charges
Any additional charges for services not included in the consultation will be advised to patients before they are requested by the clinician. An invoice detailing these charges will be raised during the consultation and will be ready for payment before you leave the Practice.
The prices for individual items are also available through our website or by contacting our reception.
If you have any queries or questions regarding fees please do not hesitate to ask for clarification. Please note additional fees may be incurred for vaccinations, laboratory fees, procedure fees, etc. All such fees are payable immediately after the provision of service.
3. New patient Health Questionnaire
Before your first appointment, we will ask you to fill out a Health Questionnaire. For your safety, this must be filled in as accurately as possible including the details of all your existing medication. To complete this Health Questionnaire, please log into your member's portal here, go to Questionnaires and complete the questionnaire. Once completed, this will be submitted through the system and will be saved in your patient notes ready for the Doctor to review with you at your first consultation.
If you do not have access to the internet or need assistance completing the Health Questionnaire, please let a member of our staff know and they will be able to assist you when you arrive at the Practice. Please ensure that you arrive earlier than your scheduled appointment time to ensure that we have time to help you complete this ahead of your appointment.
When a child is registered the details of those with parental responsibility will also be sought and the parent will need to complete the Health Questionnaire on behalf of the Child.
4. Video Consultations
If you choose a video consultation, please note that you will be required to download the Meddbase Telemedicine Application to your phone, tablet or PC prior to your first consultation. Once you have made a video consultation appointment you will be sent an email and text confirming the appointment along with a link to join the consultation.
Please ensure that you have downloaded the app before the start of your scheduled appointment time so that this does not delay the start and leave you with less time for your consultation. It is the patient’s responsibility to ensure that this application is downloaded in advance and Fylde Private Medical cannot be held responsible for missed or delayed consultations.
The day before your telemedicine consultation our staff will call each patient to remind them of this requirement.
5. Home Visits
Our Home Visit Catchment area is based on a 15-minute drive time radius from our Practice. Only by pre-agreement are home visits available to those who live within this catchment area.
Based on availability, home visits may be agreed with the practice outside of this radius. However, extra travel time will be charged at a rate of £65 for Doctors and £24 for Nurses per 30 mins, minimum charge of 15 mins.
Please note that home visits are suitable for patients not well enough to come into the surgery.
6. National Health Service (NHS) care
Members are encouraged to continue their registration with an NHS GP if they are entitled to NHS care. The clinic will liaise with any other healthcare professionals involved in your care, with your consent, as needed.
7. Changes to your Personal Details
Changes to your name, address, telephone number or Bank Account details must be notified to Fylde Private Medical immediately. You can log into the Patient portal and make these changes yourself alternatively you can call us during the Practice opening hours and we can make these changes on your behalf.
8. Matters outside Fylde Private Medical’s reasonable control
While Fylde Private Medical seeks to meet the service needs of Members at all times, its resources are finite and this may not always be possible. Fylde Private GP shall not be liable for service failures where Fylde Private GP is faced with circumstances outside its reasonable control.
9. Exclusion of liability for loss of profit etc
Fylde Private Medical shall not, in any event, and to the extent permitted by law, have any responsibility for any increased costs or expenses, for any loss of profit, business, contracts, revenue or anticipated savings or for any special, indirect or consequential losses incurred as a result of or in connection with any service, whether resulting from tort (including negligence or breach of statutory duty), breach of agreement or otherwise. For the avoidance of doubt, nothing in this clause or these Terms and Conditions shall exclude or restrict Fylde Private Medical’s liability for negligence resulting in death or personal injury.
10. Enforcement of Terms and Conditions
Failure to enforce or non-reliance on any of these Terms and Conditions by Fylde Private Medical will not prevent Fylde Private Medical from subsequently relying on or enforcing them. None of the Terms and Conditions or benefits of Membership is enforceable by anyone else other than you.
11. Interpretation: Use of English law
Your Membership and these Terms and Conditions are governed and should be interpreted by the laws of England and Wales.
12. Fair Usage Policy
In order to enable Fylde Private Medical to offer an efficient and rapidly accessible service to all Members, we need to ensure that excessively high usage by a minority of members is avoided.
Therefore, in a few exceptional circumstances, where usage by a member becomes excessively high, Fylde Private Medical reserves the right to discuss the level of usage with the member. This may lead to additional fees being charged for subsequent consultations during the membership year and a higher membership fee being charged for subsequent membership years. The new fee will be advised to you prior to renewal.
13. Use of your personal information
Fylde Private Medical is registered with the Information Commissioner’s Office and complies with the regulations associated with the Data Protection Act.

14. General rights to refuse service
Fylde Private Medical reserves the right to refuse to provide advice or consultations where:
· Membership fees have not been paid
· Outstanding charges have not been paid
· In its reasonable opinion, the giving of service would involve any breach of the law
· It reasonably considers that you or anyone accompanying you, is behaving or has behaved in an abusive or threatening manner to Fylde Private Medical’s employees or agents
· It reasonably considers that that you, or anyone acting on your behalf, is behaving, or has behaved, in an abusive or threatening manner to Fylde Private Medical’s employees either over the phone or in writing
Please note: if you are refused service by Fylde Private Medical you have the right to an explanation in writing (see ‘Compliments and complaints’ for contact details).
15. Changes to Terms and Conditions
Fylde Private Medical is entitled to change any of the Terms and Conditions on the anniversary of your joining date. Fylde Private Medical also reserves the right to make changes to these Terms and Conditions during the Membership Year, on the giving of reasonable notice, where it reasonably considers this necessary in order to comply with any applicable laws, regulations or the instruction of a regulatory authority.
16. Annual Membership Plan Terms and Conditions
i. Introduction
Welcome to Fylde Private Medical and thank you for purchasing your annual Membership Plan.
These Terms and Conditions are valid for Members joining on or after 1st October 2021. Please read this information carefully and keep it in a safe place as use of your Membership Plan is subject to these Terms and Conditions.
Please note that whilst most of the Terms and Conditions apply to all Membership Plans, there are some variations depending on the type of Plan you have with us. To work out which of the Terms and Conditions apply to your Membership Plan, please make sure you are aware of the type of Plan you hold.
ii. Annual Membership Plans
This section details the services that are available under the different Membership Plan options.
Annual Membership Plans: the contract is for 12 months, and the cover is paid for on a monthly basis, by Debit or Credit Card via Stripe, on the same day of the month that your membership started. The initial contract is for 12 months but thereafter runs on a continuous basis until cancelled. The minimum term prior to cancellation for annual membership plans is 6 months.
If you do not wish to renew your Annual Membership, or if you wish to switch to a different Membership Plan, you should notify Fylde Private Medical prior to your renewal date. Please see further details on the cancellation policy.
We offer both Individual and Family Membership Plans. Please note that Family memberships are restricted to the same membership plan type i.e. all members included within a family membership must all either be Essential, Premium, Platinum, Total, Total Plus or Corporate Standard. For the purposes of our family membership, a household is defined as patients living at the same address but can include students at university.
A Child is defined as under the age of 18 years old. If your child turns 18 years of age during the membership year, they will be updated as an adult in our system at the next annual renewal date and the increased rate applied.
17. Premium Health Plan
i. The following entitlements are included per patient per year:
Service Scope and Limitations
• Consultation Entitlement: Members are entitled up to 8 GP consultations (face-to-face or telephone) per membership year.
• Appointment Duration: Standard consultations are strictly 15 minutes. Issues requiring more time may necessitate booking an additional inclusive session or paying a supplementary fee.
• Acute Blood Test Bundle: FBC Full blood count, ESR inflammation, CRP infection or inflammation produced by the liver, Urinalysis UTI’S, D-Dimer to excluded blood clots. (1 each per year)
• Annual Active Health Check: including blood tests, ECG and full detailed report. (1 each per year)
• Specialist Assessments: The annual skin assessment is for identification only. Any subsequent diagnostic procedures, biopsies, or treatments are excluded and will be quoted separately. (1 each per year)
• Specific Cancer Screening: PSA (men) and CA125 (women) blood tests are limited to one each per year.
• Exclusions: These memberships do not cover emergency (A&E) services, chronic condition management requiring secondary care, or the cost of private prescriptions/medication.
• Priority Access: Priority access to same day/next day appointments is subject to clinical availability and "Best Endeavours".
Membership and Subscription Terms
Pricing:
• Single: From £55pm (6 appointments)
• Couple: From £95pm (6 appointments each)
• Family (up to 4): From £165pm (6 appointments per person)
• Large Family (4+): Capped at a total of 24 appointments per year for the entire group.
18. Platinum
Annual Membership Plan - £95 per month – single membership
i. The following entitlements are included per patient per year:
· Free initial joining GP Consultation – 30 minutes (once only)
· Free In-Clinic (face-to-face) or telephone GP or Nurse consultations during opening hours (4 included per person per year
· Free Video consultations during opening hours (3 included per person per year)
· Free GP or Nurse home visits during opening hours within our catchment area (2 included per person per year)
· Free annual GP Led health MOT – 1 hour (includes tests)
· Free private prescriptions (medication charges apply)
· Free referrals to private specialists (consultation and treatment costs not included)
· Appointments on the same day or within 24 hours
· Up to 30-minute consultations
· Online patient booking portal
· Preferential access to our diagnostic services (MRI/CT/Ultrasound) at our local partner’s facility
· 15% discount on additional consultations over above those included in your annual plan
· 15% discount on blood tests and diagnostic services
· 15% discount on health screenings and medicals
· 15% discount on medical procedures and all our services
· Quarterly practice e-bulletin news
· Out of hours messaging service
· No joining/membership fee
ii. We are pleased to be able to offer the following family Platinum membership discounts:
· Couple - £150 per month
· Child - £35 per month (under 18 years old)
· Family of 3 - £185 per month
· Family of 4 - £220 per month
· Family of 5/6 - £250 per month
iii. Family membership is based on two adults and children
19. Premium Annual Membership Plan - £65 per month – single membership
i. The following entitlements are included per person per year:
· Free initial joining GP Consultation – 30 minutes (once only)
· Free In-Clinic (face-to-face) or telephone GP or Nurse consultations during opening hours (3 included per person per year)
· Free Video consultations during opening hours (3 included per person per year)
· Free private prescriptions (medication charges apply)

· Appointments on the same day or within 24 hours
· Up to 30-minute consultations
· 10% discount on GP or Nurse home visits within opening hours within our catchment area
· 10% discount on additional consultations over above those included in your annual plan
· 10% discount on blood tests and diagnostic services
· 10% discount on health screenings & medicals
· 10% discount on medical procedures and all our services
· Online patient booking portal
· Quarterly practice e-bulletin news
· Out of hours messaging service
· No joining/membership fee
ii. We are pleased to be able to offer the following family Premium membership discounts:
· Couple - £110 per month
· Child - £25 per month (under 18 years old)
· Family of 3 - £135 per month
· Family of 4 - £160 per month
· Family of 5/6 - £185 per month
iii. Family membership based on two adults and children
iv. Home Visits

20. Essential Annual Membership Plan - £35 per month – single membership
i. The following entitlements are included per patient per year:
• Free In-Clinic (face-to-face) or telephone GP consultations during opening hours (3 included per person per year) 15-minute consultation times
• Free Video consultations during opening hours (2 included per person per year) 15- minute consultation times
• Free private prescriptions (medication charges apply)
• Free referrals to private specialists (consultation and treatment costs not included)
• Appointments within 2 working days
• 5% discount on medical procedures and all our services (excluding Aesthetics)
• Online patient booking portal
• No joining/membership fee
• Out of hours messaging service
ii. We are pleased to be able to offer the following family Premium membership discounts:
· Couple - £65 per month
· Child - £15 per month (under 18 years old)
· Family of 3 - £80 per month
· Family of 4 - £95 per month
· Family of 5/6 - £110 per month
iii. Family membership based on two adults and children
21. Appointment Cancellation Policy (Annual Membership Plans)
You can cancel or reschedule appointments up to 24 hours before the appointment. You can do this free of charge through the patient portal or by calling the Practice on 01253207568 during our opening hours.
If you cancel the appointment within 24 hours or do not attend, you will lose one of your included annual consultations.
If you reschedule the appointment within 24 hours you will incur an administration fee of £15.
22. Missed Appointments (Annual Membership Plans)
If you do not cancel or do not attend the appointment without informing us you will lose one of your included annual consultations from your membership plan
23. Cancellation of Annual Membership Plans
i. You have the right to cancel your Membership Plan within a 14 day ‘cooling off period’, commencing from the date of your membership and initial payment. Cancellations must be confirmed in writing.
The following refund policy will apply for cancelling within the cooling-off period:
ii. If you have already had a consultation, you will receive a full refund of the total Membership fee paid less the charges for the consultation.
iii. If you have not had a consultation, you will receive a full refund of the fee, less an administration charge of £30.
iv. Please note that there will be no additional cooling-off period(s) during the Membership Year, regardless of any changes that are made to the Membership Plan.
If you wish to cancel after the cooling-off period has expired the following will apply:
v. The minimum term prior to cancellation for annual membership plans is 6 months. There will be no refund of the Membership fee for the first six months. The only exception will be in the event of death, in which case we may refund your estate with an amount proportionate to any full months left.
vi. You can cancel your cover at any time after the initial 6 months by giving a minimum of 30 days notice. Payment cancellation will take effect at the next payment date following the end of the notice period. You will be advised of the date when your membership expires.
vii. Fylde Private Medical has the right to cancel any membership plan immediately if you or anyone accompanying you, is behaving or has behaved in an abusive or threatening manner to Fylde Private GP employees or agents, or if, in Fylde Private Medical’s opinion the therapeutic relationship has irretrievably broken down or it is clear that the service expectations and demands of the patient are unreasonable in relation to the services provided by Fylde Private Medical
In addition, a Membership Plan can be cancelled by Fylde Private Medical:
viii. Immediately if any month’s Continuous Membership fee or other related charge is overdue;
ix. At any time by giving at least 30 days notice: cancellation will take effect at the next payment date following the end of this notice period and no refund will be due.
24. PAY AS YOU GO (PAYG) Terms and Conditions – In addition to the General Terms and Conditions (points 1 – 15) the following apply:
i. These Terms and Conditions are valid for Patients using our service on a Pay-As-You-Go basis on or after 01/10/2021. Please read this information carefully and keep it in a safe place as use of our services are subject to these Terms and Conditions.
ii. Pricing
Please see our website for our Pay As You Go Pricing. If you require additional information or have any questions, please contact the Practice and we will be happy to help you.
iii. Appointment Cancellation Policy
You can cancel or reschedule appointments up to 24 hours before the appointment. You can do this free of charge through the patient portal or by calling the Practice on 01253 207568 during our opening hours.
If you cancel the appointment within 24 hours you will be charged 50% of the full price of the consultation.
If you reschedule the appointment within 24 hours you will incur an administration fee of £15.
iv. Missed Appointments
If you do not cancel, or do not attend the appointment without informing us you will be charged the full price of the consultation.
